Part of our patient advocacy services is proactively monitoring your medical chart and looking for potential errors that can occur. What can sometimes appear to be a minor error, such as an inaccurate weight documentation, can result in significant consequences. Many high risk medications, such as chemotherapy and blood thinning drugs, are dosed based upon your weight. The accuracy of that weight is critical to ensure you are not over or under-dosed.
